All Avondale Heights Football Club Players who have paid their club membership and are fully financial, are covered under Bronze level membership  with JLT SPORT (Australian Football National Protection Program)

This cover is paid for by the Avondale Heights Football Club under arrangement with the EDFL and in the interests and welfare of the players. The cover provides basic financial assistance for Non-Medicare Medical Benefits.

If a player needs to make a claim, download the claim form from the link above and fill out the application. The club declaration is to be signed by club secretary. Follow directions in forwarding form to JLT.

PLAYER WELFARE

The Avondale Heights Football Club does not cover any player medical liability, private insurance, dental or ambulance cover and strongly recommends that all players are members of their own health fund, ambulance fund and wage protection insurance.

 

Some levels of private health insurance may include ambulance cover.

The Avondale Height Football Club understand the importance of risk management and strongly recommend that all players take out “Work Protection Insurance”

 

The AHFC is not liable for the cost of ambulance cover or medical expenses or for the loss of wages due to injuries.

By being a player with the Avondale Heights Football Club, you give the club permission to attend to any first aid treatment deemed necessary? This may include the contacting of emergency services and /or transport to a hospital. All ambulance cost and medical expenses is to be paid by the player.

For injury management, discuss with club sports trainers, consult your own medical practitioner , or health worker (Physiotherapist, Osteopath, etc)
